This was done through the introduction of a new version called the J.
The new option
According to the J. Joint Declaration, a joint declaration may be provided as a replacement for a marriage certificate during a passport or extension process. The new form is especially useful for people wishing:
Add the name of the spouses to the passport
Change your last name after marriage
App J components
To fill the application form J, the applicant needs to provide the following:
Joint photo couple
Signatures of both husband and wife
Dates of birth of both partners
The numbers of both of both
Either the ID of the voter or the passport
In case of divorce or death of spouses
It is worth noting that in order to remove the name of the former spouses, the divorce decree or the death certificate is still necessary.
In case of re -marriage, a new marriage certificate or a joint declaration with the current husband and wife will be required.
